Enrollment Agreement

This contract is entered into between Feldenkrais Manhattan Teacher Training Program, hereinafter referred to as the “FMTT” and

Individual named hereinafter referred to as “Trainee.”

1. The Trainee shall receive at least 12 Functional Integration® lessons from experienced Feldenkrais trainers, assistant trainers and practitioners as part of the training. The cost of these lessons is included in the tuition. FMTT will organize the scheduling of these lessons to take place on the program premises.
2. Tuition will be $5400.00(USD) per year (200 hours of training). The Trainee acknowledges by his/her signature at the end of this Contract that he/she will abide by the terms of the Tuition Payment Schedule. Furthermore, the Trainee agrees that he/she shall promptly inform the Administrative Director in the event that he/she is unable to fulfill his/her financial obligations to FMTT. Unless prior arrangements have been made with the Administrative Director, no Trainee shall be allowed to continue with a Training Program segment unless the tuition due has been paid in full.
3. FMTT will be an in-person training which will take place over 800 hours during a period of not less than three- and one-half years. Important but ancillary to the in-person program curriculum, we will organize online study as appropriate.
4. In an ongoing process the Educational Director, trainers and assistant trainers will evaluate the Trainee on the basis of class attendance, participation, homework, and supervised in-class Awareness Through Movement and Functional Integration practice.
5. FMTT reserves the right to terminate the Trainee from the program at any time. Reasons for dismissal may include but are not limited to: noncompliance with the rules and directives of FMTT, disturbance of the group training process, exhibiting difficulties in the personal and/or professional integration of the learning material, and/or nonpayment of tuition fees according to the Tuition Fee Schedule. When FMTT identifies a reason for possible Trainee termination, the Trainee will be notified and possible solutions to the problem will be explored before a final determination is made regarding the Trainee’s continuation in or termination from the program. In the event that the Trainee is terminated from the program, FMTT shall refund to the Trainee all tuition payments for the training sessions of the program which the Trainee has not yet attended at the time of termination. Such refund shall not apply to days missed before termination.
6. FMTT Administrative Director or staff will track the Trainee’s attendance at the training to ensure that the Trainee has participated in the required number of hours. In the event of any absence from class the Trainee is responsible for the content of the missed material. The Trainee must, as soon as possible, make up all the missed time with the assistance of the Educational Director. The Trainee may miss no more than five (5) days in any year, and no more than ten (10) days during the whole course of FMTT without having to make arrangements with the Educational Director to make up missed class time. If the Trainee exceeds these permitted absences, he/she may be required to attend makeup segments in other training programs, at additional cost to the Trainee. The training has to be completed within a maximum of seven years, and a minimum duration of 3.5 years.
7. The Trainee understands that he/she will be eligible to receive authorization from the Educational Director and FGNA to teach group Awareness Through Movement® classes, on a provisional basis, after satisfactory completion of the second year of the FMTT. Satisfactory completion must include teaching Awareness Through Movement while supervised by a member of the educational staff. The Late Enrollment Trainee understands that the point at which he/she will be eligible for authorization to teach Awareness Through Movement® classes, on a provisional basis, may be as late as the completion of the third year of the training. The trainee agrees not to advertise at all during the first two years of the training. If the trainee drops out or is terminated from FMTT after two years of enrollment, and before graduation, he/she will lose the right to teach Awareness Through Movement® unless a written Leave of Absence from the Educational Director has been provided to FGNA.
8. Upon successful completion of the training, the graduate will be qualified and eligible to receive authorization from FGNA to become a Guild Certified Feldenkrais PracitionerCM and to teach Awareness Through Movement lessons and individual Functional Integration lessons. The Trainee will receive a Diploma from FMTT, which may be accepted by Feldenkrais Professional Associations or Guilds around the world.
9. The Trainee intends to complete the FMTT full training program. If the Trainee wishes to transfer to another Feldenkrais Method Training Program, he/she will make the request in written form to the Educational Director. The Educational Director will then advise the Trainee as to the options available for transfer. If the Trainee transfers to another program, their training must be completed within seven years of the start date of this particular program.
10. FMTT makes no claims or warranties concerning the uses or applications of the movement lessons and/or procedures taught in this course other than that the course is fully accredited by, and organized under the policies of THE FELDENKRAIS GUILD® of North America (FGNA®) an international accreditation board, and that the course curriculum satisfies the basic requirements for accreditation by The FGNA.FMTT does not and cannot guarantee that graduates of the FMTT will be able to practice in countries, states or counties where laws, licenses, and regulations may stipulate additional specific requirements. Graduates must apply to FGNA for initial certification, and must renew certification annually.The FMTT and FGNA offer no guarantee of internship or employment during the training period or following graduation from a training program.
11. By signing this contract, the Trainee authorizes FMTT to provide his/her name, address, phone, and email to FGNA® and authorizes FGNA to use this information to contact the Trainee.

13. The Trainee agrees to comply with all rules and directives of the program, and agrees that they are subject to and will comply with the FGNA Code of Professional Conduct and Grievance Protocol and Feldenkrais Method Standards of Practice, and requirements for use of FGNA service marks, as they may change from time to time.
14. The Trainee acknowledges that this training is organized and accredited by the North American Training Accreditation Board under internationally approved policies In the U.S. and Canada, the terms Feldenkrais®, Feldenkrais Method®, Functional Integration®, Awareness Through Movement®, ATM^SM, FI^SM, L’intégration fonctionnelle^SM, Prise de conscience par le mouvement^SM, Friends of Feldenkrais^SM Guild Certified Feldenkrais Teacher®, Guild Certified Feldenkrais Practitioner^CM, Feldenkrais Awareness Through Movement Teacher^CM, Feldenkrais Guild® and the Feldenkrais® logo may only be used with permission from and following the guidelines of the Feldenkrais Guild® of North America, which are available at https://www.feldenkraisguild.com/page/service-mark-use or by contacting servicemarks@feldenkraisguild.com. In other countries, service marks related to the practice of the Feldenkrais Method must be used in accordance with guidelines established by the owner(s) of these marks.
15. Trainees will be accepted into the program on a provisional basis for the first two weeks. During this period the Trainee can get to know the program and the educational staff can get to know the Trainee. If the educational staff feels that the Trainee cannot realistically complete the program, the Trainee will be given a complete refund and all agreements will be terminated.
16. Trainees desiring graduation may be required to do outside-of-class work, including reading, written assignments, observation of Functional Integration done outside of class hours, practice of Awareness Through Movement and Functional Integration in between training segments, and additional mentoring in some cases if deemed necessary by the Educational Director.
17. The Trainee understands that graduation is not granted automatically after the end of the training.
18. The undersigned agrees to indemnify and hold harmless Sheryl Field, dba Feldenkrais Manhattan, against any damages, claims, liabilities and expenses arising from the actions or omissions of the undersigned, the training program, any employees or contractors or sub-contractors associated with the training program. The undersigned agrees to indemnify and hold harmless Sheryl Field, dba Feldenkrais Manhattan, from any loss or perceived loss due to any changes in the structure of the training program, such as staffing, venue location, or Feldenkrais Guild of North America requirements for graduation and accreditation.
19. The Trainee confirms that a separate FMTT enrollment application document has been completed, dated, signed and submitted.
20. The Trainee confirms that a separate document outlining the Tuition payment agreement had been provided, completed, signed and submitted.
21. By signing this contract, the Trainee acknowledges that he/she/they understand and accepts all terms and conditions of this contract and agrees to abide by all such terms and conditions.
